Yep John, to much heel trimming and not enough attention to details......

I bet that hoof is high inside hence the lateral wall crushing and heel becoming under run. This will affect the fit and the problem becomes self perpetuating.

He looks fetlock Varus Gary, juvenile development issue I recon. Farrier thought he was probably doing the horse a favor.
It just shows perfectly that not only is the skill required to fit these shoes but a good bio mech knowledge to decide if its going to cause more trouble.
I didnt think the heels looked to bad level wise.
The capsule usually distorts crushing the outside heel, when the leg tracks to the mid center line.
That would be usually medial distal tarsal pain. If you force the leg back out you criple the hock, the horse is putting the leg there to avoid pain. As Mr P says straight bar helps to prevent shearing and further capsule distortion.
Also the loaded twist as Mr P says is usually stifle, quite often has that two stage loading of the hock in conjunction.
Both can have PSD as a secondary for obvious reasons, but very difficult to diagnose with other conditions.
    But I might be wrong.

yes, find this a lot here. have a few on for now; then pull off a few shoeing later; it is working on this particular horse at the moment.

You think the shoe is racked because the medial heel is too long? You're the first person I've heard of fitting shoes starting at the heel.

Yes the medial heel on that shoe is to long. If the medial heel of the shoe was a little shorter then the toe of the shoe would be in the right place & not off centre to the frog . How did you come up with you're the first person I've heard of fitting shoes starting at the heel. From me saying the medial heels to long ?o_O & what does this racked mean ?

yes, correct; and a lot of farriers I see rack every hind shoe. Racked means: what I have been told and seen.....Jack mac is when you turn the shoe out so that the inside toe nail is higher than the outside toe nail pulling the whole shoe to one side and then the inside heel is shorter than the outside heel by up to 1/2 inch difference. I had a shoe like that; I "might" still have it and need to get pic of it; I'll send it to someone who can then post here to show what I mean. very popular here to do; but not my style.
always fitted my shoes starting with the heel.:cautious:

yes see what you mean; and that is light way of racking and being off center like that.
I have seen it up to 1 inch difference and do it every horse they shoe.

your point is well taken; agree with you.

It is a relatively simple matter(if you are adept at drawing lines with the computer, a feat at which I am not accomplished)to show a line that actually bisects the red dot , tip of the frog and then the frog spline. IOW, I think the line shown is inaccurately placed.

I also feel the line is incorrectly placed. The shoe is not racked.
